What is meant by putting the 12V clamp on--What is this clamp device? If it is the hold-down clamp for the battery, then it may be putting force on the battery case to cause an internal shorting of the battery plates or current collector bars.

Also what is the age and condition of your 12V battery? That is something to consider when troubleshooting issues. An old weak or worn out 12V battery can cause a multitude of issues with ECUs and relays, although usually a DTC is thrown for a low voltage fault.
The inductors and transformers inside a Mitsubishi OBC (made by Nichicon) get hot; to charge faster would require an upgrade in sizing and capacity of these magnetics in addition to the cooling system. Tesla OBC is larger and has better cooling system too.
Use your very expensive meter and measure directly across the 12V battery terminals with the Key in OFF position, that will give you the open circuit voltage. If your 12V battery is the original OEM from when the car was new, then you have gotten your money's worth and then some. Congrats.
