Loss of power (limp mode). Outlander 2007

Hi has anyone had the same experience with there outlander I have owned mine a couple years and it's constantly going into limp mode nearly every time I drive it, it mainly happens when I'm accelerating but I never go over 3 thousand revs as if I do it happens straight away??
