Questionnaire

Has anyone else received an independent questionnaire about their new car.
There are questions about the car, the dealer, your previous car, reasons for choosing the PHEV, what you like and dislike, any faults, etc. etc.

I'm told by our dealer that there is a government questionnaire that we are expected to complete in order to qualify for the £5000 grant - is this something different? I'm still waiting to see the one that the dealer is referring to.
